Root cause: daemon E2E tests read user's real ~/.claude/settings.json which has Bash(rm:*) in allow list, causing rm commands to auto-execute without permission prompts. Direct claude-agent.test.ts works because it uses useTempClaudeConfigDir() to create isolated settings with ask: ["Bash(rm:*)"] and sets CLAUDE_CONFIG_DIR env var. Fix: Add temp config setup to daemon tests (same pattern as direct tests) or use settingSources: [] for SDK isolation mode. # Plan
|
||
|
||
## Context
|
||
|
||
Build a new Codex MCP provider side‑by‑side with the existing Codex SDK provider. The new provider lives in `packages/server/src/server/agent/providers/codex-mcp-agent.ts` and is selected via a new provider id (e.g. `codex-mcp`). All testing is **E2E only** (no mocks/fakes). Use `/Users/moboudra/dev/voice-dev/.tmp/happy-cli/src/codex/` as reference for MCP + elicitation.

||
|
||
## CRITICAL RULES - READ BEFORE EVERY TASK
|
||
|
||
1. **NO VAGUE REPORTS**: Never say "test hung", "was interrupted", "failed locally" without:
|
||
|
||
- The EXACT error message or stack trace
|
||
- The SPECIFIC line of code causing the issue
|
||
- A concrete hypothesis for the root cause
|
||
|
||
2. **NO SKIPPING/DISABLING TESTS**: Skipping tests, adding `.skip`, or "opt-in gating" is **NOT ACCEPTABLE**. Fix the actual problem. If a test hangs, find out WHY and fix the code, not the test.

|
||
3. **NO WORKAROUNDS**: Adding timeouts, fallbacks, or "defensive" code that hides bugs is forbidden. The code must work correctly, not appear to work.

4. **INVESTIGATE DEEPLY**: When something fails:

For our `codex mcp-server` flow, elicitations only fire on exec approval requests; with `approval-policy=untrusted` the safe-command allowlist (e.g., `pwd`) bypasses approval, so no `elicitation/create` is emitted.

RESULT: Claude permission allow/deny/abort flows resolve correctly and MCP agent run completes without timeouts; create_agent no longer fails schema validation. RESULT: Bug verified as fixed - clicking/opening agent does NOT update timestamp, only actual interactions (sending messages) update it. Server only sets `agent.updatedAt` in `recordUserMessage` (`packages/server/src/server/agent/agent-manager.ts:436`) and `handleStreamEvent` (`packages/server/src/server/agent/agent-manager.ts:864`), not in `clearAgentAttention` or `initializeAgent` flows. In contrast, direct tests use `useTempClaudeConfigDir()` (`claude-agent.test.ts:57-87`) which creates a temp config dir with `ask: ["Bash(rm:*)"]` and sets `CLAUDE_CONFIG_DIR` env var. EVIDENCE: User's `~/.claude/settings.json` contains `"Bash(rm:*)"` in allow list. SDK uses `settingSources: ["user", "project"]` (`claude-agent.ts:665`) to read settings from disk. FIX: Add temp config setup to daemon tests (same pattern as direct tests) or use `settingSources: []` for SDK isolation mode. Full report: `REPORT-claude-permission-tests.md`.
