Mohamed Boudra 5b32d0a56c fix: update MCP tests to use createAgentSelfIdMcpServer ...

Tests for set_title and set_branch were looking for these tools in
mcp-server.ts but they were moved to agent-self-id-mcp.ts during
the MCP split.
