fix(opencode): show model details on v2 subagent rows (#7799 )

The v2 child descriptor omitted the session's presentation facts, leaving completed children with title-only rows. Reuse the existing subtitle formatter for both live updates and reconciliation. The compact regression came from unmounting the sheet while its visibility tracker survived in Combobox. Its non-closed phase blocked the next present(). Keep the tracker inside the sheet component so they unmount together, and preserve the desktop closed-state guard in Combobox.
