Add DaemonClient permission methods and Codex E2E permission tests

- Add `extra` field to CreateAgentOptions for provider-specific config
- Add Codex permission tests (approve/deny) - both passing
- Add Claude permission tests (skipped - SDK doesn't request permissions
  in daemon context despite correct config)
- Full permission cycle verified: permission_requested → respondToPermission
  → permission_resolved → tool executed/denied
