* fix(opencode): reconnect v2 sessions after helper exits

A helper exit aborted the session lifetime, so every later prompt failed even though OpenCode can serve the same native session from another helper. Keep the session open, fail the active turn, and acquire and configure the current helper on the next OpenCode operation.

* fix(opencode): forget exited v2 helper generation

A dead shared helper could remain selected as the current generation. Clear that selection at exit so the first lazy reconnect acquires a live helper.
