* fix(pairing): keep terminal QR codes scannable

Prompt framing could wrap dense QR codes and alter long pairing links. Print pairing instructions directly, add the standard quiet zone, and suppress codes that cannot fit without auto-wrapping.

* fix(pairing): contain terminal QR background
