* Notify a caller once when it prompts a child that is still running

create_agent and a background send_agent_prompt each armed their own
finish notification for the same child and caller. When the second
prompt replaces the child's running turn, the child never goes idle in
between, so its one finish fired both and the caller got the same
notification twice. Arming again for a child and caller now replaces
the earlier notification.

Closes #5397
