Update 15-provider test expectations to match current product state:
- Provider count: 3 → 5 (added copilot, pi)
- Claude models: 3 → 4 (added claude-opus-4-6[1m])
- Codex models: replace retired gpt-5.1-* with gpt-5.4/gpt-5.4-mini
