Mohamed Boudra 888b54bb44 feat(hosts): resolve qualified agent addresses through shared runtimes

Keep agent identity and origin authority stable through disconnects, destination changes, and scoped disposal. Share the resolver with trusted daemon consumers and plugins, using ordinary sessions and per-admission permission ceilings. Expose MCP host discovery through that boundary as the first checkpoint toward durable fleet subagents.
