* fix(server): notify the caller when a blocking send_agent_prompt outlasts its wait

An agent-scoped send_agent_prompt with background: false only armed the
finish notification for background sends. When the 30s wait timed out,
the tool returned "still running" and nothing was armed, so the child's
result never reached the caller. The wait now reports whether it timed
out, and a timed-out wait on a still-running agent arms the same
notification a background send gets and says so in guidance. A blocking
send that finishes inside the wait still returns its result without a
notification.

* refactor(server): declare the finish-notification helper as a function
