* fix(claude): restore each background task notification once Queued and delivered copies of one completion had different synthetic tool identities. Prefer their shared task ID so existing timeline projection folds them into one notification. * fix(claude): preserve separate notifications from one task A task can emit several Monitor events, including identical text.
