Claude Code reports a turn that stopped on an API error (a session or usage limit, an invalid key, an overloaded request) as a `success` result with `is_error: true`, the error text in `result`, after a synthetic assistant reply carrying the same text and an `error` field. The provider read only the subtype, so the turn completed with the error as its answer: the agent went idle with no lastError and showed as finished in the sidebar. Treat such a result as a failed turn with the result text as its error. Hold the synthetic error reply back until the result arrives so the failure row does not repeat it; a reply the turn moves past, or a result without is_error, still shows it.
