paseo-bot[bot] 2187124642 Answer an ACP agent's open permission request when its turn fails (#6045 )

* fix(acp): answer open permission requests when a turn fails

When an ACP prompt failed while one of its session/request_permission
requests was still open, Paseo stopped showing the request but never
answered it, so agents that refuse new prompts until the request is
settled stayed stuck until the agent was reloaded.

A turn that ends without completing now answers its open permission
requests with cancelled, as interrupt and close already do.

Closes #6040

* test(acp): cover permission requests left open by a cancelled turn
